A Sustainability and Transformation Plan (STP) for Health and Social Care Services in Norfolk and Waveney has been submitted to NHS England.
All local NHS are required by NHS England to produce a Sustainability and Transformation Plan (STP) outlining how they working to ensure that they are able to provide safe, sustainable local services. (For background information about STPs please click here).
The submission is an assessment of the challenges faced by the local health and social care system and an outline proposal for some measures to help address them through continued work with residents and patients. It is a work in progress and further staff, public and patient engagement will help shape the emerging detailed plans.
