Are you a member of a community organisation or group that helps local people to stay healthy and well? Do you have an idea for a new service or activity but don’t have the resources to set it up? If so, West Norfolk CCG would like to hear from you.

West Norfolk CCG is launching a new fund to encourage and support innovative ways of helping local people to remain independent within the community.

Funding will be released annually, with a different theme for each year. This year, the focus will be on initiatives that support frail, elderly people in West Norfolk.

Applications that demonstrate changes that will support frail, elderly people to maintain their independence so that they have less need for statutory health and social care services (such as the Queen Elizabeth Hospital and care homes) are particularly encouraged.

It is expected that grants will normally be issued within the range of £2,000 – £5,000.

We are open to a wide range of ideas about how best to support frail, elderly people and have provided some additional supporting information so that you are aware of existing and planned services that the CCG is currently leading, to inform your application:
