Expressions of Interest Invited to review the Transitions Service

Transitions is a project for Young Adult Carers (16-24 year olds) in Norfolk funded by the Big Lottery. Launched in April 2013 in Great Yarmouth, Norwich and Kings Lynn it was expanded to Thetford and Attleborough with an additional grant from Comic Relief in April 2014. The service’s current funding ends in April 2016 and Norfolk Carers Support hopes to use this evaluation to demonstrate the difference we have made and the continued need for a dedicated outreach service to Young Adult Carers.
