Community Action Norfolk are seeking to appoint voluntary community and social enterprise (VCSE) providers to deliver Social Prescribing in the West Norfolk and Breckland areas. 

Social Prescribing, sometimes referred to as “community referral”, is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services.

Recognising that people’s health is determined primarily by a range of social, economic and environmental factors, Social Prescribing seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.

Social Prescribing is being developed across Norfolk. In the West Norfolk and Breckland District Council areas Community Action Norfolk is managing the delivery of Social Prescribing.

As part of this Community Action Norfolk is seeking to appoint VCSE organisations to deliver the day to day activity of Social Prescribing through hosting ‘Living Well Workers’.
