NHS Sustainability and Transformation Plans

NHS England requires that local NHS produce two separate but connected plans:

  • A five year Sustainability and Transformation Plan (STP), which drives the Five Year Forward View and is place-based
  • A one year Operational Plan for 2016/17, which is organisation-based but consistent with the above mentioned emerging STP

CAN is currently working to ensure that these plans will incorporate effective engagement with the voluntary, community and social enterprise sector and will circulate more information, when it becomes available.

Background:

NHS England are asking all health and care systems to come together to create their own local plan for accelerating the implementation of the NHS Five Year Forward View.  (To read the NHS Five Year forward View, please click here.)

STPs will work towards the following three aims:

  • Better health
  • Transformed equality of care delivery
  • Sustainable finances

The process should involve local communities and voluntary, community and social enterprise sector organisations, building on the six principles from the Five Year Forward Plan:

  • Care and support is person-centred, personalised,coordinated and empowering
  • Services are created in partnership with citizens and communities
  • Focus is on equality and narrowing inequalities
  • Carers are identified, supported and involved
  • Voluntary, community and social enterprise and housing sectors are involved as key partners and enablers
  • Volunteering and social action are recognised as key enablers
