Social Prescribing in West Norfolk and Breckland

Norfolk and Waveney’s Sustainability and Transformation Partnership plans include the objective of developing Social Prescribing across Norfolk.

Through discussion with local partners in the Breckland District Council and King's Lynn and West Norfolk Borough Council areas (all of West Norfolk CCG and the Breckland part of South Norfolk CCG) Community Action Norfolk will be managing the delivery of Social Prescribing.

VCSE organisations took part in a competitive process to be selected as to be a provider under Social Prescribing. As a result of this process seven providers have been appointed. Each will employ one full time equivalent Living Well Connector. Connectors will work closely with colleagues in primary and social care to deliver Social Prescribing.  Each provider has been allocated to a GP cluster. Providers will primarily support clients who live within that cluster however, all providers will work together to manage case loads and make use of the specialist expertise of each organisation. Providers and current cluster allocation:

  • Age UK Norfolk - Breckland/Ketts Oak
  • Family Action - Swaffham
  • Future Projects - Breckland
  • Home Group Limited- Fens
  • The Matthew Project- Mid Norfolk
  • West Norfolk Carers - Coastal
  • West Norfolk Mind- Kings Lynn

Our next steps

  • We are in the process of recruiting staff to deliver the service
  • We are working towards Living Well Connectors being in place by the 2nd July 2018
  • We expect service launch date to be soon after this, once all appropriate induction and training elements have been completed.
  • We are working with NCC and other partners on confirmation of referral processes, more detailed information about the service provision that can be distributed to partners and specifics around some of the evaluation elements.
